APPLICATION REQUIREMENTS

Note:

1. Print a working copy of all forms.
2. Gather all materials needed to complete the forms.
3. Enter your data onto each of the online forms. (Each form must be completed entirely - data cannot be saved!!)
4. Print each of your completed forms, sign them, and mail or bring them into Mound City Bank.
   w  Current financial statement (Inventory Worksheet)
   w  12 month cash flow projection (Agriculture Analysis Form)
   w  General Credit Application
   w  Past 3 years income tax returns
   w  Additional information may be required upon request
